Premier’s Value-based Care Consulting and Collaboratives: Best in KLAS 2022, 2023 & 2024

Connecting People, Data and Knowledge

Hospitals, health systems, clinically integrated networks and healthcare professionals work with Premier’s PINC AI™ Strategic Collaboratives to support their organizations on the journey across the value continuum.

A national collaborative networking program aimed to enable its participants to drive improved levels of value for patients and hospitals. The Collaborative is an innovative approach to support improved performance in the Hospital Value-Based Purchasing, Pay-for-Performance and other Centers for Medicare and Medicaid Services (CMS) programs with an ultimate goal of achieving the 100 Top Hospitals® designation.

Affinity Groups for Academic COOs, CQOs, CMOs and CNOs engage with peer stakeholders in networking, knowledge sharing and exploration of innovations to address challenges in today’s healthcare environment. These groups collectively take on projects that include addressing health equity, new models of care delivery and the workforce crisis.

Successfully implement bundled payments through robust program and episode management. Leverage claims data within Premier’s best in class analytics tool, the Bundled Payment Intelligence Platform (BPIP), to optimize cross-continuum care, monitor performance against benchmarks, and improve provider engagement. Accelerate improvement by working with peers across the nation.

We support Medicare models (Bundled Payments for Care Improvement Advanced (BPCIA), Comprehensive Care for Joint Replacement (CJR), Enhancing Oncology (EOM)), Shadow Bundles in MSSP, as well as non-Medicare areas, such as Centers of Excellence models.

Influence future policy through innovative analytics, assessments, and knowledge sharing, and meet CMS requirements for Health Equity and Social Determinants of Health. Advancing the work of Health Equity is a key priority of our health system leaders, payers, hospital associations and professional societies. Members in the Health Equity Collaborative are collectively leading the way in improving the delivery of care for all patients.

Convene with national leaders in healthcare to provide medical practices with new options to thrive in today's turbulent times. Drawing input from current medical practice executives, the collaborative blends data sharing, transparency, policy advocacy, shared learning and cohorts to rapidly address and advance the needs of the academic, regional and national health systems. Members work together to disrupt and innovate from within, focus on provider resilience, evaluate the need for market segmentation strategy and collaborate with peers for change.

The Population Health Management (PHM) Collaborative provides a framework to support member organizations in implementing successful and sustainable strategies and tactics in population health models across payers.

PHM Collaborative members consistently outperform the nation in total cost of care models. Members taking part in the Medicare fee-for-service shared savings model initiatives have received a greater prevalence of shared savings than non-Collaborative member ACOs across the duration of the programs. Notably, the Collaborative provides support to drive commercial, Medicare Advantage, and Medicaid success as well, and actively leads innovative and progressive national employer-based care delivery initiatives with the potential to have a significant impact on our healthcare landscape.

A multi-year national collaborative representing more than 500,000 births per year.
