Convene with national leaders in healthcare to provide medical practices with new options to thrive in today's turbulent times. Drawing input from current medical practice executives, the collaborative blends data sharing, transparency, policy advocacy, shared learning and cohorts to rapidly address and advance the needs of the academic, regional and national health systems. Members work together to disrupt and innovate from within, focus on provider resilience, evaluate the need for market segmentation strategy and collaborate with peers for change.
The Population Health Management (PHM) Collaborative provides a framework to support member organizations in implementing successful and sustainable strategies and tactics in population health models across payers.
PHM Collaborative members consistently outperform the nation in total cost of care models. Notably, the Collaborative provides support to drive commercial, Medicare Advantage, and Medicaid success as well, and actively leads innovative and progressive national employer-based care delivery initiatives with the potential to have a significant impact on our healthcare landscape.
